INSURANCE

 

Dr. Goldstein is presently not contracted with any insurance companies, and as such is considered an out-of-network provider.

If you have a PPO insurance plan and have met your deductible, it is likely that a percentage of the charges will be reimbursed to you at the out-of-network rate. In addition, all services will apply towards qualified medical expenses if you have a Health Care Reimbursement Account (HCRA) or a Health Savings Account (HSA). 

 

We will provide an itemized superbill that includes appropriate CPT (procedure) and ICD 10 (diagnostic) codes for you to submit to your insurance for reimbursement after you have made a payment to our office.

FEES FOR FUNCTIONAL MEDICINE

Most PPO insurance companies reimburse for initial consultations and subsequent visits.  Most of them also at least partially reimburse for necessary lab tests.   Some but not all PPO insurance companies reimburse for telemedicine or virtual visits. 

 

Functional medicine visits are billed as evaluation and management, not as chiropractic care.

Most PPO insurance companies will reimburse you for the initial consultation and examination and for periodic re-examinations upon receiving your superbill.  Most PPO’s also reimburse for physical rehabilitation (exercise, postural reprogramming, soft tissue work) and chiropractic treatment.

 

If your chiropractic care is related to injuries sustained in a motor vehicle accident, we may be able to bill your auto insurance directly, if you have MedPay included in  your auto policy.
